What is This Old Tool & How Was it Used?

It look like a combination lock wrench and hammer. You twist the handle and the lock wrench section on each end (smaller on the thinner end) locks the object into that open head area.
I'm pretty sure that's how that works, but can't figure out what it would have been used for. Or what it's called
It looks like it grabs a square nut.
That suggests to me it might be a wrench for removing plumbing plugs and caps? Something a plumber might use? Those plugs have a square fitting. And the two different ends are for different size plugs?
That is my guess.
